Service Description

This 75-minute intake session is intended to gain pertinent background information and presenting concerns in order to best serve you. At the end of the intake session, we will identify 2-3 goals that we will target in counseling. You will receive a detailed treatment plan before your next session. Intake sessions can be done either in person or by telehealth. ***The INTAKE SESSION must be scheduled prior to any individual counseling sessions!*** ***SUBJECT TO APPROVAL BASED ON AVAILABILITY*** ***PHONE CONSULT MUST TAKE PLACE PRIOR TO INTAKE SESSION***
